This includes patients attending as follow-up from a hospital admission as per Medicare rules.

Any issues in addition to those listed on your referral will require a new referral to be provided

A detailed discharge summary is usually provided on discharge from hospital to facilitate continuation of care by your GP should you wish not to follow-up in clinic

• Please bring an up to date list of your medications or the medications themselves
• Please bring copies of any relevant results that you may have

Dr Magoche does NOT provide Emergency or Urgent Care through this outpatient clinic. If your condition deteriorates and you are in need of urgent care please call 000, or present to your nearest Emergency Department. Alternatively, if you do not feel you are critically unwell, you may opt to contact 1300Health or your GP.
